CDSMP My Life, My Health

This program is for adults who are either living with the daily challenges of one or more ongoing health conditions or caring for someone with these conditions. Participants will be introduced to new tools they can use to assume an active role in their self-management program.

Duration: 6 weeks, 2 ½ hour sessions

Diabetes Self-Management Program

This interactive program acknowledges that there is more than one way to manage Diabetes. Participants will gain new tools to help build and carry out a plan that fits their life.

Duration: 6 weeks, 2 ½ hour sessions

Chronic Pain Self-Management Program

A program for those who have chronic pain. Learn techniques and exercises to help reduce frustration, fatigue, isolation and improve strength, flexibility and endurance

Duration: 6 weeks, 2 ½ hour sessions

Healthy Eating for Successful Living in Older Adults

Want to learn about nutrition and how lifestyle changes can promote better health? This AARP sponsored program will provide nutrition education, goal setting and problem solving based on the USDA My Plate model!

Duration: 6 weeks, 2 ½ hour sessions

A Matter of Balance

This program is for adults who experience the fear of falling. Learn how to make realistic goals for increasing activity, exercises to increase strength as well as balance, and changing environmental factors to reduce the risk of falling.

Duration: 8 weeks, 2 hour sessions

Memory Training

Learn what memory is, how it works, how to reduce tip of tongue moments, how to reduce your memory burden. Improve your learning style and better memory habits. Learn strategies and techniques to remember names/faces, where you put things and much more!

Duration: 4 weeks, 2 hour sessions

Building Better Caregivers

Caregiving can be an extremely demanding job, and many Caregivers experience isolation and burnout. Some of the most important aspects of becoming a better Caregiver are learning how to take good care of yourself by managing your own emotions, stress and physical health.

Duration: 6 weeks, 2 ½ hour sessions

Honoring Choices Massachusetts

This program is designed to help individuals 18 years or older understand and utilize their right to make their own health care choices. Participants will gain tools and information to help them develop their personal health care plan for a time when one is unable to communicate his/her desires for treatment.

Duration: 1 to 2 hours

Fall Prevention

Do you have a history of falls? In this program you will learn about remaining independent, fall statistics, fall risks, home safety and fall prevention through exercises. Receive handouts and a safety checklist!

Duration: 1 ½ hours

Healthy Eating On a Budget

Using the USDA Choose My Plate, learn how to navigate the nutrition food label,  menu planning and clever ways to eat healthy on a tight budget while setting realistic goals.

Duration: 1 to 2 hours

Dementia Friendly Massachusetts

Anyone can become a Dementia Friend. The goal is to help everyone in a community understand the five key messages about dementia, how it affects people, and how we each can make a difference in the lives of people living with the disease.

Duration: 1 hour

Tai Chi - Moving for Better Balance

Tai Chi is a graceful form of exercise that is suitable for older adults. Tai Chi involves low impact exercise that puts minimal stress on muscles and joints. Tai Chi is recommended to prevent falls among older adults.

Duration: 8 weeks, 1 to 1 ½ hour sessions.
