Behavioral Health Outreach for Aging Populations

Through this program, behavioral health clinicians provide (or we could say complete) assessments and develop care plans that may include individualized counseling, care coordination to community resources, and referrals. This behavioral health program comes right to the consumer and works with older adults in their home, or at a location that is comfortable for the consumer with the goal of improving their overall well-being.  If you are feeling challenged, don't know where to turn to for support, please consider giving us a call or filling out our online referral form.
